By checking this box, I acknowledge and agree to the following:
- I grant permission to Rise 8 Basketball Academy to use my photograph(s) and/or video recordings taken at any Rise 8 event, program, or activity for any lawful purpose, including but not limited to promotional materials, social media, websites, advertisements, and publications.
- I understand that these materials may be edited, altered, copied, or distributed without further approval from me. I waive any right to royalties or other compensation arising from the use of my image.
- I release Rise 8 Basketball Academy and its representatives from any claims, demands, or liabilities in connection with the use of the images or recordings.
- I understand that this release is irrevocable and that I have no control over future use of the materials once they are published.
- If the individual photographed or recorded is under 18 years old, I certify that I am the parent or legal guardian and grant permission on their behalf.
